Advanced care planning ensures medical treatment received by patients is consistent with their wishes regardless of their ability to communicate. Recognizing the direct impacts on patient care and disruption to families that result from inadequate documentation of end-of-life care planning, AllSpire established the Advanced Care Planning (ACP) Initiative. This initiative uses a team approach to educate hospital staff, community members and patients about end-of-life care planning that includes improved documentation of patient wishes and ensures clinical implementation. The ACP team includes Palliative, Hospice and behavioral healthcare leaders from each AllSpire health system.
